Occupational Consultants are experts in providing hospital discharge planning for both workplace and catastrophic/ complex injuries. The team at Occupational Consultants will liaise with the client, treating team and family to ensure their safety and needs are met on discharge.
Dependent on the injury and clients needs discharge planning can include pre and post discharge assessments. A pre discharge assessment may be undertaken to review access to the home and ensure equipment recommendations are viable within the home environment.
A post discharge assessment may include a review of equipment, transfers (bed, toilet, chair, car), existing supports, cognition, personal care and other daily living needs.
The Occupational Therapist will assist in arranging/ obtaining equipment recommendations and referrals to short term services such as domestic or personal support.
